Home Manager
Duties and Responsibilities
- • Provide ongoing mentoring, coaching, and training to all leads and direct care staff
- • Complete debriefing on all incident reports
- • Coach staff on appropriate ways to write Incident Reports (IR)
- • In-Servicing Plans
- • Lead weekly Lead Trainings and monthly Staff Trainings
- • Create annual performance appraisals for every staff member
- • Oversee the daily duties and responsibilities of all staff in the home
- • Oversee the use of proper CPI physical management skills
- • Flexibility and commitment to work the hours and times required to perform the responsibilities of the job, including evenings and weekends as may be necessary
- • Maintain a positive relationship with management and staff
- • Maintain and advocate for all Flatrock residents’ rights
- • Maintain inventory of activity supplies
- • Act as a Community Liaison
- • Create and oversee schedule on daily basis that is appropriate to needs of residents
- • Promote home and community interaction
- • Daily check of resident floor documentation (including SDR, check in/check out, progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation as indicated in the plan) for completion and accuracy
- • Scan SDRs daily and upload daily once they are complete and accurate
- • Train floor staff and new hires on floor documentation process to meet Coordinated Care Professional Service standard of excellence
- • Review progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation at the end of each month and sign off on its completion and accuracy
- • Scan documentation and upload it to FIN (due on the 10th of each month)
- • Create or revise individualized SDRs as new residents move in or as plans change year to year
- • Complete tracker with documentation errors noted daily
- • Complete tracker with documentation of staff training dates on each piece of documentation
Floor Documentation
- • Daily check of resident floor documentation (including SDR, check in/check out, progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation as indicated in the plan) for completion and accuracy
- • Scan SDRs daily and upload daily once they are complete and accurate
- • Train floor staff and new hires on floor documentation process to meet Coordinated Care Professional Services, Inc. standard of excellence
- • Review progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation at the end of each month and sign off on its completion and accuracy
- • Scan documentation and upload it to FIN (due by the 8th business day of each month)
- • Create or revise individualized SDRs as new residents move in or as plans change year to year
- • Complete tracker with documentation errors noted daily
- • Complete tracker with documentation of staff training dates on each piece of documentation
Lead Documentation
- • Monitor and train daily documentation including but not limited to:
- ◦ Shift notes, lead staff checklist, radio logs
- ◦ All IRs that happen in the home. It is the floor manager’s responsibility to ensure follow up has happened for each IR including debriefing
- ◦ Written coaching documentation
Plans
- • Attend in-servicing for all plans.
- • In-service and train staff providing direct care to residents on all resident plans (IPOS, BTP, and OT)
- • Complete in-service sign-in forms with new hires, or in-service new plans or added plans with present staff
- • Complete binder, adding new plans or added plans when they become available for in-home orientation with new hires/transfers
- • Update/train staff when changes are made to plans/behavioral interventions and ensure that revisions are implemented in a timely manner
- • Initiate an email to the team when a plan is 45 days from expiration as a reminder that the plan is expiring, and new plans are needed. Monitor email communication until a new plan is delivered
- • Complete in-service trackers with in-service dates and plan expiration dates
Communication
- • Communicate with clinical team regarding resident’s progress, plans, and concerns
- • Utilize all communication tools including Microsoft Teams, text and phone, email, and Flatrock Information Network (FIN)
- • Communicate with guardians, case managers, and others who share responsibility for our residents
- • Responsible for ensuring Flatrock Inc. residents arrive at their medical appointments on time
Implementation
- • Assist the Psychologist, LLP, and BCBA in duties associated with achieving excellence in the implementation and monitoring of residents’ Positive Behavior Supports Plans and assisting in psychological services
- • Monitor implementation of behavioral interventions through direct observation
- • Provide feedback to staff, and re-train staff as needed, to ensure excellence in the adherence to the implementation of the Positive Behavior Supports Plans as written.

May involve travel to other locations/agencies/meetings. May require evening, weekend, and holiday hours.
Qualifications
- • High School Diploma or GED
- • 1+ years of care home experience (recommended)
- • 1+ years of leadership experience (recommended)
- • Present or current experience with behavioral health issues
