Program Assistant Manager

Duties and Responsibilities

  • • Daily check of resident floor documentation (including SDR, check in/check out, progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation as indicated in the plan) for completion and accuracy.
  • • Scan SDRs daily and upload daily once they are complete and accurate.
  • • Train floor staff and new hires on floor documentation process to meet Coordinated Care Professional Services, Inc. standard of excellence.
  • • Review progress notes, token charts, hygiene charts, ABC charts, inclusion logs, and any other documentation at the end of each month and sign off on its completion and accuracy.
  • • Scan documentation and upload it to FIN (due on the 10th of each month).
  • • Create or revise individualized SDRs as new residents move in or as plans change year to year. 
  • • Complete tracker with documentation errors noted daily.
  • • Complete tracker with documentation of staff training dates on each piece of documentation.
  • • Attend in-servicing for all plans.
  • • In-service and train staff providing direct care to residents on all resident plans (IPOS, BTP, and OT).
    • ◦ Complete in-service sign-in forms with new hires, or in-servicing new plans or addended plans with present staff.
  • • Complete binder, adding new plans or addended plans when they become available for in-home orientation with new hires/transfers.
  • • Update/train staff when changes are made to plans/behavioral interventions and ensure that revisions are implemented in a timely manner.
  • • Initiate an email to the team when a plan is 45 days from expiration as a reminder that the plan is expiring, and new plans are needed. Monitor email communication until a new plan is delivered.
  • • Complete in-servicing trackers with in-service dates and plan expiration dates.
  • • Assist the Psychologist, LLP, and BCBA in duties associated with achieving excellence in the implementation and monitoring of residents’ Positive Behavior Supports Plans and assisting in psychological services.
  • • Monitor implementation of behavioral interventions through direct observation.
    • ◦ Provide feedback to staff, and re-train staff as needed, to ensure excellence in the adherence to the implementation of the Positive Behavior Supports Plans as written.
  • • Maintain a positive, open relationship with all management and staff
  • • Utilize all communication tools including FIN
  • • Maintain and advocate for all Flatrock Inc. residents’ rights  
  • • Complete any additional tasks assigned from the Psychologist, BCBA, and LLP’s

Qualifications 

  • • 1+ years of care home experience (recommended) 
  • • 1+ years of leadership experience (recommended)
